Referrals

Although patients may self refer to the clinic at any time, we welcome clinicians’ referrals as well.

We applied for and will hopefully soon hold a GP Focused Practice Special Designation in Addiction Medicine with the Ontario Medical Association and MOHLTC committee.

We are now offering Buprenorphine subdermal implant (Probuphine^T) insertion and removal.

To refer a patient, please fax us a referral letter containing reason for referral and patient’s contact information at fax number: 613-354-0093. We will contact the patient within the next days.
